How to set up a MyChart account.

Create and access your MyChart account

Patients 13 years and older can create a MyChart account.

Request a proxy account

Parents and legal guardians can request proxy access to their child or teen's record by signing into their own MyChart account and going to the Friends and Family Access page to submit a Proxy Access Request form.

If you have an activation code from your doctor

  1. Look for an activation code on your After Visit Summary or in an email or text message from your healthcare organization.
  2. Follow the instructions provided with your activation code to create your MyChart account.
  3. Go to MyChart
  4. Enter your:
    • Activation code
    • Last four digits of your Social Security number
    • Date of birth
  5. Create a username and password.
  6. Confirm your email address (for alerts only).
  7. Read and accept the terms and condition.

Access your MyChart account by logging in via a web browser or with the free MyChart app.
